- Go to evicore.com*.
- Click either Providers or Resources at the upper right.
- Click the Provider Resources button.
- Select Blue Care Network in the “Select Health Plan” field.
- Click the Search button.
- Click the Solution Resources tab.
- On the Blue Care Network Provider Resources page, click the Musculoskeletal button.
- Click the link to open the desired document.
